-DMULTITOS flag in MiNT

It was my understanding that the -DMULTITOS flag for MiNT was only to
indicate that the `INIT=' program was a GEM application.  However, when I
made a -DMULTITOS version of MiNT, it ignored all the applications after
MINT.PRG in the AUTO folder and seemed to behave differently in general. 
Could someone please explain what the -DMULTITOS flag is really for?
