CAB: these forms crash

These 3 forms either crashed CAB or resulted in several
error messages. Could anyone else give them a try?

My system:  CAB 2.5 & MiNTnet.OVL 1.81

The problem usually involves forms which uses the same URL
for generating successive parts of a survey form.
